Getting in Touch with UMI

Referring a patient to UMI is easy. Our scheduling office is open from 8:00am until 6:00pm to help arrange appointments that are convenient for your patients.

Once the appointment has been scheduled, simply download, print and complete the UMI Online Requisition Form. Please fax the completed form to (585) 341-9066 and give your patient a copy to bring to the appointment.

To obtain a consult or request medical records, our medical records staff is available to assist you or your staff with any needs you may have.
Digital Reporting
To deliver the diagnostic results your patients expect, you need true round-the-clock image access. Click here to find UMI’s Digital Reporting gateway page, or, if you’re already registered and familiar with the ImageCast system, you may log in and retrieve digital reports here.
Patient Education Literature
This printer-friendly document is available for you to download and distribute as needed to your patients as they prepare for a diagnostic test at UMI. It is intended to answer most of their questions about imaging modalities; the logistics of making an appointment, coming to our facility, and contact information; and our total commitment to patient and physician service.
