When the time comes to refer a patient to a local imaging facility, we hope you will consider the expertise and experience of University Medical Imaging. Our radiologic specialists, state-of-the-art technology and seasoned support staff are all at your disposal.

For your convenience, we have created a self editing online requisition. Please complete electronically, print, sign and fax to our attention. You may also save a copy to your documents for your records.

Once you have chosen UMI for your patients’ imaging needs and completed a requisition, feel free to contact our office yourself, or pass on our number to your patient so that he or she may schedule an appointment.
UMI radiologists are on site and available to consult with you and your patients about any issues that may arise before, during or after an appointment. With regard to scan data, we are committed to fast completion of reports, and immediate image availability via a secure Internet portal. (Our policy is to notify your office at once, via telephone, should abnormal or otherwise urgent findings be discovered.)
